When submitting a change request please include the page on which the change is needed.  The more details you provide will make it much easier.

REMEMBER TO

Include the following in the "Request Details" section of the form

1) The page / URL of the page that require the changes

2) What changes are required

NOTE: If you are looking to change to your phone number, address, or email address please make sure they are also updated with MyLCI (This can be done by the club secretary)
